Supports Coordination

Supports Coordination (SC) is a service provided to consumers with developmental disabilities, as well as to consumers with mental illness who only need basic services to maintain progress and further their recovery. The Supports Coordinator assists consumers in utilizing community resources and natural supports (family, friends, volunteers, etc.) to best help them realize needs and desires. Supports Coordination promotes independence and stability by providing services that are based on consumer strengths, needs, abilities and preferences.

Supports Coordinators meet with consumers in their homes or in the community with a frequency based on consumer need. Supports Coordinators are available 24 hours per day for crisis management.
