Case Management

Case Management provides clinical case management services to adult consumers with severe and persistent mental illness. Case managers make home visits and provide consumers with community contacts that can assist them in areas such as medication monitoring, education on mental illness, access to community resources, and vocational and recreational activity development. Case Management provides supports that allow consumers to be as independent as possible within their communities. Case Management also helps in reducing the number, intensity and frequency of psychiatric hospital days.

After-hours coverage includes crisis intervention and, if needed, medication monitoring to prevent hospitalization. Case Management consumers who have an after-hours emergency may contact the case manager on-call 24 hours per day.
