Senior Case Manager

Job Description:

Working in collaboration with the team supervisor, will train and coach staff in methods to effectively manage and maintain a caseload, best practice protocols including transition, acute care protocols, and jail services, etc. Instruct team on practices and procedures relating to consumers with dual diagnoses. Will take on case load as indicated by consumer census and team need. Assist on handling crisis or emergency events to facilitate smooth transition and effective interventions. Provide back-up coverage of Supervisor as needed. Participate in rotating on-call schedule and creates the monthly on-call schedule for the team staff. Must have knowledge of co-occurring modalities including the ability to engage, screening, motivational interviewing and treatment matching protocols.

Preferred Skills:

Ideal candidate must be able to multi-task in a fast-paced environment. Masters in social work with appropriate credentials/ licensure required. LLMSW or LMSW preferred. Supervisory experience a plus.
